Prospective Stuides of Hemodynamic and Arterial Blood Gas Hanges with CO2, Gas Handling during Laparoscopic Cholecystectomy

Bae Sung-Han
Abstract
A prospective evaluation of 23 patients undergoing laparoscopic cholecystectomy during a 2 months from Aprii 1992 through May 30, 1991 was performed, We were evaluated the cardiovascular effect and blood gas changes during laparoscopic surgery
with
CO2
insufflation and deflation times periods.
The measurements of cardiac output, cardiac index, systemic vascular resistance, mean artrial blood pressure, heart rate, central venous pressure, end tidal PCO2 ad blood gas analysis were studied in 23 patients who underwent laparoscopic
cholecystectomy with CO2 handling during procedure. This study was done 10 minutes after endotracheal intubation(control), 30 minutes after CO2 insufflation, 60minutes after CO2 deflation, and 6 hours after CO2 deflation except end tidal PCO2. We
also
compared the arterial blood gas was done before anesthesia as a control.
@ES The results were as follows:
@EN 1) End tidal POC2 was increased after CO2 insufflation.
2) Arterial PCO2 was increased from initial CO2 insufflation to 60 minutes after deflation and then was recovered in 6 hours after deflation.
3) Arterial PO2 was not decreased.
4) mean arterial pressre was increased after CO2 deflation and 6 hours after CO2 deflation was changes to control values respectively.
5) Central venous pressure was increased CO2 insufflation and was normalized to control values.
6) Cardiac output, cardiac index was slight increased after Cow insufflation and CO2 deflation times.
7) Systemic vascular resistance was not changes throught the all preocedure.
